Please read the information below before beginning the application process for Northwest High School:

  1. Does the parent/legal guardian of the student live in the Northwest Community Schools district? If yes, please proceed; if no, please contact the Central Administration Office at (517) 817-4726 for the Schools of Choice or District Release options.
  2. If you do not have an email address, please come to the school office for an enrollment packet.
  3. Create a login and password to start the enrollment process.
  4. Print out your completed application and attach the required documentation.
  5. Please contact Northwest High School Guidance Office at (517) 817-4820 to schedule an appointment to complete the enrollment process.

REQUIRED DOCUMENTATION FOR ENROLLING A NEW STUDENT

WHEN ENROLLING A NEW OR TRANSFERRING STUDENT, PLEASE CALL THE NORTHWEST HIGH SCHOOL GUIDANCE OFFICE FIRST TO SCHEDULE AN APPOINTMENT WITH THE APPROPRIATE COUNSELOR. THE FOLLOWING ITEMS MUST BE BROUGHT WITH YOU TO YOUR APPOINTMENT:

    1. ORIGINAL BIRTH CERTIFICATE
    2. CURRENT IMMUNIZATION RECORDS
      1. 4 doses of DTP or TD with the most recent TD (Tetanus/Diptheria/Pertussis) shot being within the last 5 years.
      2. 3 doses of Polio
      3. 2 doses of MMR (Measles, Mumps, Rubella)
      4. 3 doses of HBV (Hepatitis B)
      5. 2 doses of Varicella (Chicken Pox) or present decumented immunity from a reliable source. This could be a parent's statement that their child has previously had Varicella Disease.
      6. 1 dose of Meningococcal (Meningitis)
    3. PROOF OF RESIDENCY (need to provide all 3):
      1. Mortgage, Lease, or Rental Agreement (a Property Tax Statement is also acceptable if you own your home)
      2. Current utility bill (phone, electric, garbage, cable, etc.)
      3. Current credit card bill, bank statement, car insurance, etc.
    4. UNOFFICIAL TRANSCRIPT FROM PREVIOUS SCHOOL, WITHDRAWAL GRADES (if during the school year), AND IEP (for special education students)

If you are unable to provide all of the required documentation noted above, please contact Northwest High School prior to your scheduled appointment.

Please Note: Your student will not receive a schedule or be allowed to attend school until a principal has contacted the previous school and authorized his/her enrollment. You will then receive a telephone call from the Guidance Secretary or Counselor to notify you of the student's start date.
